Flat feet are not automatically a problem. Many people have low arches and remain completely comfortable. The concern is when the arch changes shape, the foot rolls in excessively, or pain begins to limit walking, work, exercise or the shoes you can wear. Understanding how to fix flatfoot pain starts with identifying what is actually being overloaded – because a painful flat foot is not one single diagnosis.
Pain may sit along the inside of the ankle, through the arch, beneath the heel, in the ball of the foot or even in the knee and lower back. For some people, symptoms settle with appropriate footwear and a structured rehabilitation plan. For others, progressive tendon failure, arthritis or a fixed deformity needs specialist assessment before the condition becomes harder to treat.
Why flat feet can become painful
The arch of the foot is supported by bones, ligaments, joints and tendons. One of the most important is the posterior tibial tendon, which runs behind the inside of the ankle and helps support the arch during walking. When this tendon becomes irritated, stretched or torn, the arch may gradually collapse and the heel can drift outwards.
This is often called adult-acquired flatfoot or progressive collapsing foot deformity. It can develop gradually, particularly in middle age, after an injury, with increased body weight, inflammatory arthritis, diabetes, or years of repetitive loading. Some people have naturally flexible flat feet from childhood and only become symptomatic when their activity, footwear or tendon capacity changes.
A painful flat foot can also be caused by arthritis in the hindfoot or midfoot, ligament injury, a tight calf muscle, previous fractures, or a rigid structural foot shape. These distinctions matter. A soft, flexible flat foot is managed differently from a stiff foot with arthritic joints.
How to fix flatfoot pain: begin with the right assessment
Before choosing an orthotic or exercise program, consider the pattern of your symptoms. Pain on the inside of the ankle, swelling around the posterior tibial tendon, difficulty standing on one foot or an arch that is visibly dropping are reasons to arrange a clinical assessment. A specialist can examine foot alignment, tendon strength, joint movement, calf tightness and the way you walk.
Weight-bearing X-rays are often useful because they show the foot under load, rather than only when it is resting. Depending on the findings, ultrasound, MRI or CT imaging may be considered to assess tendons, ligaments, joint arthritis or more complex deformity.
This process is not about rushing towards surgery. It is about establishing whether the pain is coming from a tendon, joint, ligament or pressure point, then matching treatment to the stage of the condition. Treating a progressive deformity as though it were simple muscle fatigue can delay effective care.
Practical measures that may reduce symptoms
Choose shoes that support the foot rather than fight it
Supportive footwear is often the most useful first change. Look for a firm heel counter, a stable sole that does not twist easily, enough depth for an insole if needed, and a secure fastening such as laces, straps or a structured slip-on design. The shoe should feel stable through the heel and midfoot, not merely soft underfoot.
Very flat, flexible shoes, worn-out runners and unsupportive sandals can allow an already overloaded foot to roll in further. This does not mean every person with flat feet needs a stiff shoe all day. Comfort, work requirements and the degree of deformity all matter. However, when symptoms are active, a stable shoe can reduce strain while other treatment takes effect.
Use orthoses or bracing when clinically appropriate
Over-the-counter arch supports can help mild, flexible symptoms, especially when combined with suitable footwear. They may reduce pressure beneath the arch and improve comfort during walking. If the foot is significantly pronated, the deformity is progressing, or a tendon is painful, a custom orthosis or an ankle-foot brace may offer more meaningful support.
An orthosis is not designed to permanently rebuild an adult arch. Its purpose is to control excessive motion, redistribute load and make activity more tolerable. It should also be reviewed if it causes rubbing, numbness or worsening pain. The right device depends on your foot shape, shoes, occupation and activity level.
Build capacity gradually with targeted exercise
When the posterior tibial tendon and surrounding muscles are irritated but intact, a carefully prescribed strengthening program can be valuable. Exercises commonly focus on calf flexibility, balance, controlled heel raises and strengthening the muscles that support the arch and ankle.
The key is dosage. Pushing through sharp pain or dramatically increasing walking, running or gym activity can prolong tendon symptoms. A useful rule is that discomfort should remain mild and should not cause a significant increase in pain or swelling later that day or the following morning. A physiotherapist or podiatrist can tailor loading to your diagnosis and current capacity.
Modify activity without stopping all movement
During a painful flare, temporarily reducing hills, long walks, high-impact sport or prolonged standing may be sensible. Cycling, swimming or other lower-impact exercise may allow you to maintain fitness while the foot settles. Complete rest for extended periods, however, can weaken the very muscles and tendons needed for support.
If your work keeps you on your feet, changes may be practical rather than dramatic: more supportive work shoes, regular sitting breaks, avoiding repeated stair climbing where possible, and pacing tasks across the day. Small changes can make a noticeable difference when the foot is being loaded for hours at a time.
Manage inflammation thoughtfully
Ice after activity may ease short-term soreness and swelling for some people. Simple pain relief or anti-inflammatory medication can be appropriate for certain patients, but should be discussed with a pharmacist, GP or treating clinician, particularly if you have stomach, kidney, heart or blood pressure conditions, or take other medication.
Pain relief can help you stay mobile, but it should not be used to mask a worsening deformity. If the arch is visibly changing or ankle weakness is developing, assessment is more useful than repeatedly treating symptoms alone.
When conservative care is not enough
Non-surgical treatment is often successful for early, flexible flatfoot pain. It may need several months of consistent footwear changes, orthotic support and progressive rehabilitation before improvement is clear. The appropriate timeframe varies according to the tendon involved, the severity of pain and whether arthritis or deformity is present.
Surgery may be considered when pain remains function-limiting despite well-directed conservative treatment, when the foot continues to collapse, or when there is a rigid deformity and arthritic joint pain. The aim is not simply to create a higher arch on an X-ray. Reconstructive surgery seeks to restore a more stable, functional foot and address the structures responsible for pain.
Procedures vary considerably. In a flexible deformity, treatment may involve tendon reconstruction, ligament repair, calf-lengthening procedures and carefully planned bone realignment. In a rigid or arthritic foot, joint fusion may be the more reliable way to relieve pain and provide stability. The best option depends on imaging, clinical examination, overall health, work demands and your goals for walking and activity.
A surgical discussion should include recovery time, periods of non-weight-bearing or protected weight-bearing, footwear planning, expected rehabilitation and potential risks. Clear planning is particularly important for people who live alone, care for others, drive for work or need to navigate stairs at home.
Signs you should seek prompt specialist advice
Arrange assessment sooner if pain and swelling persist along the inside of the ankle, you cannot perform a single-leg heel raise, the foot has changed shape, or you are struggling to walk normal distances. A rigid flat foot, new pain after injury, numbness, skin breakdown or a warm, swollen foot in someone with diabetes also warrants timely medical review.
Sudden severe pain, inability to bear weight, fever, a red hot foot, or a wound should be assessed urgently. These symptoms may indicate a problem beyond routine flatfoot pain.
