A burning sole that becomes more noticeable after a long day on your feet is easy to dismiss as tiredness or footwear trouble. Yet tarsal tunnel symptoms can signal pressure on an important nerve around the inner ankle. Recognising the pattern early helps patients seek the right assessment before pain, altered sensation or weakness begins to limit work, walking, exercise and sleep.
What is tarsal tunnel syndrome?
The tarsal tunnel is a narrow passage on the inside of the ankle. It sits behind and below the bony prominence known as the medial malleolus. Within this space, the posterior tibial nerve travels alongside tendons and blood vessels before dividing into smaller branches that supply sensation and muscle function in the foot.
Tarsal tunnel syndrome occurs when this nerve is compressed, stretched or irritated. The condition is sometimes compared with carpal tunnel syndrome at the wrist, but symptoms in the foot can be less predictable. Some people experience discomfort only with activity, while others notice burning or pins and needles when resting at night.
The underlying issue may be inflammation, swelling, an injury, a structural foot shape, a cyst or another space-occupying lesion. In some cases, a clear cause cannot be identified. This is why a careful clinical examination matters: nerve symptoms have several possible sources, and treatment depends on locating the problem accurately.
Common tarsal tunnel symptoms
Symptoms usually affect the inside of the ankle, heel, sole or toes. They may begin gradually and fluctuate for weeks or months. A person may feel relatively comfortable in the morning, then develop a deep ache, tingling or burning as standing and walking accumulate through the day.
Typical symptoms include pain or tenderness behind the inner ankle, shooting or electric sensations into the arch or toes, numbness in part of the sole, and a burning feeling under the foot. Some patients describe the unpleasant sensation as walking on a hot surface or having a stone in the shoe, even when there is nothing there.
Symptoms can be localised or widespread depending on which branch of the nerve is affected. Pain may radiate from the ankle towards the heel and forefoot, or it may be felt primarily in the arch. Pressure from tight shoes, prolonged standing, walking on uneven ground and higher-impact activity can make symptoms more apparent.
Night pain is another useful clue. When the foot is no longer bearing weight, ongoing burning, tingling or numbness can feel particularly disruptive. However, night symptoms alone do not confirm tarsal tunnel syndrome. Other nerve conditions, including problems arising from the lower back, can produce a similar experience.
Changes in strength or balance
As nerve irritation progresses, some people notice more than altered sensation. The small muscles of the foot can be affected, potentially contributing to toe weakness, cramping, reduced balance or a less secure feeling when walking. These changes are not present in every case, but they should not be ignored.
Long-standing nerve compression can be harder to reverse than early irritation. Persistent numbness, increasing weakness or a noticeable reduction in foot function warrants timely specialist review.
Why tarsal tunnel symptoms are often mistaken for other conditions
Heel pain is common, and the sole of the foot contains many structures capable of causing discomfort. Plantar fasciitis, for example, often creates pain near the heel that is worse with the first steps after rest. Tarsal tunnel syndrome is more likely to involve burning, tingling, numbness or radiating sensations, although the two conditions can coexist.
Morton’s neuroma more commonly affects the forefoot, usually creating pain, burning or numbness between the toes. Peripheral neuropathy may affect both feet in a more symmetrical pattern, while nerve irritation from the spine can travel down the leg into the foot. Arthritis, tendon disorders, stress injuries and poorly fitting footwear also need consideration.
For this reason, self-diagnosis based on one symptom can lead patients down the wrong path. The quality, timing and distribution of pain, together with an examination of the foot, ankle, gait and lower limb, provide much more useful information than an online symptom checklist.
What can cause pressure on the tibial nerve?
Tarsal tunnel syndrome has no single cause. An ankle sprain or fracture may leave swelling, scar tissue or altered mechanics around the tunnel. Flat foot posture can place additional tension on the tibial nerve, particularly when the arch rolls inward during walking. Conversely, a bony prominence, ganglion cyst, varicose vein or inflammatory tissue can physically reduce the available space around the nerve.
Conditions associated with generalised swelling or inflammation may also contribute. Diabetes and other causes of peripheral nerve disease can complicate the clinical picture, as can inflammatory arthritis. For runners, workers who stand for extended periods and people returning to activity after injury, repetitive loading may reveal symptoms that had previously been mild.
Finding a contributing cause is valuable because it shapes management. Treating inflammation, correcting contributing biomechanics or addressing a defined mass are very different clinical pathways.
When should you arrange an assessment?
An assessment is sensible when burning, tingling, numbness or inner ankle pain persists beyond a short period, keeps returning, or affects normal footwear and daily activity. It is particularly worthwhile if conservative measures such as changing shoes, reducing aggravating activity or using appropriately prescribed support have not settled the problem.
Seek urgent medical attention for sudden marked weakness, rapidly spreading numbness, an inability to bear weight after an injury, severe swelling or signs of infection such as redness, heat and fever. These symptoms may indicate a problem other than tarsal tunnel syndrome and should not wait for a routine appointment.
At a specialist consultation, the clinician will discuss when the symptoms began, where they travel and what triggers them. Examination may include palpating the tarsal tunnel, testing sensation and muscle function, observing foot posture and assessing how the ankle moves. Imaging such as ultrasound, X-ray or MRI may be used where a structural cause is suspected. Nerve conduction studies can sometimes help evaluate nerve function, though results are interpreted alongside the clinical findings rather than in isolation.
Treatment depends on the cause and severity
Early treatment often focuses on reducing nerve irritation and addressing the factor causing compression. This may involve activity modification, footwear changes, anti-inflammatory measures where clinically appropriate, orthoses to improve foot mechanics, or a period of immobilisation after an injury. Physiotherapy may be considered to address strength, mobility and movement patterns.
These approaches can be effective, particularly when symptoms are recent and a biomechanical contributor is clear. They are not a guaranteed answer for every patient. If a cyst, severe deformity, scar tissue or another physical obstruction is compressing the nerve, symptoms may persist despite otherwise well-managed conservative care.
In selected cases, surgery may be considered to release the tarsal tunnel and relieve pressure on the nerve. If another condition is responsible, such as a mass or structural deformity, that may need to be addressed as part of the surgical plan. The decision is individual: surgery has potential benefits, but recovery, wound care, temporary activity restrictions and the possibility that nerve symptoms take time to improve must be weighed carefully.
A surgical assessment should provide clear information about the suspected cause, whether non-surgical options remain reasonable, the planned procedure and the expected recovery pathway.
