The first few steps out of bed can tell a revealing story. If you put weight through your foot and feel a sharp, bruised or burning sensation beneath the heel, heel pain may be more than a passing ache. For some people it settles after moving around; for others, it affects work, exercise, footwear choices and the simple comfort of walking.
The heel is a complex load-bearing structure. Pain in this area is often blamed on a heel spur, but the source may instead involve the plantar fascia, Achilles tendon, bursae, nerves, joints or the heel bone itself. An accurate diagnosis matters because treatment that helps one cause can aggravate another.
Why heel pain is not one diagnosis
The location, timing and character of pain provide useful clinical clues. Pain under the heel, particularly with the first steps in the morning or after sitting, commonly points to plantar fasciitis. This is irritation and degeneration at the attachment of the plantar fascia, the strong band of tissue that supports the arch.
Pain at the back of the heel may relate to Achilles tendinopathy, inflammation around the tendon, a prominent heel bone, or irritation where the shoe presses against the area. In children and teenagers who play sport, pain at the back of the heel can be related to calcaneal apophysitis, also called Sever’s disease, while the growth area is still developing.
Burning, tingling or electric-shock sensations deserve particular attention. These symptoms can indicate nerve irritation, including tarsal tunnel syndrome, rather than a straightforward soft-tissue problem. Deep pain after an injury, or pain that remains severe at rest, can occasionally reflect a stress fracture or another bone-related condition.
A heel spur seen on an X-ray does not automatically explain symptoms. Many people have spurs without pain, while others have significant plantar fascia pain without a visible spur. Specialist assessment considers the full picture rather than treating an image alone.
Common causes of heel pain
Plantar fasciitis
Plantar fasciitis is one of the most frequent causes of pain beneath the heel. It often develops gradually, although a change in activity can make it suddenly noticeable. Long periods on your feet, increased running or walking, hard work surfaces, unsupportive shoes, weight changes, tight calf muscles and altered foot mechanics can all contribute.
The classic pattern is pain with the first steps after rest, followed by a degree of easing as the foot warms up. Symptoms may return later in the day after prolonged standing or exercise. It is not simply an inflammatory problem in every case, particularly when it has been present for months, which is why management needs to be tailored to the stage and cause of the condition.
Achilles and posterior heel conditions
The Achilles tendon connects the calf muscles to the heel bone. Repetitive loading, a sudden training increase, calf tightness and footwear that rubs at the heel can lead to pain and thickening within the tendon or around its attachment.
Some patients also have Haglund’s deformity, where a prominent upper-back part of the heel bone creates shoe pressure and irritation of nearby tissue. Treatment differs depending on whether pain arises from the tendon, bursa, bone prominence or a combination of these structures. Continuing high-impact activity through significant Achilles pain can increase the risk of more serious tendon injury.
Stress injury, arthritis and nerve pain
A calcaneal stress fracture may occur after a substantial increase in walking, running or standing, particularly when bone health or recovery is compromised. The heel can feel painful with squeezing from the sides and may not improve after warming up.
Arthritis in joints around the rearfoot, inflammatory conditions and nerve compression can also cause heel symptoms. These causes are less common than plantar fasciitis but should not be overlooked when symptoms do not fit the usual pattern or fail to respond to sensible early care.
When to seek assessment for heel pain
Mild symptoms that have only just started may improve with activity modification, supportive footwear and a measured approach to stretching. Persistent or recurrent pain, however, warrants a clinical review. Seeking an opinion earlier can be particularly valuable when your occupation requires standing, when you are unable to exercise normally, or when pain is changing the way you walk.
Arrange prompt assessment if pain has continued beyond several weeks despite conservative measures, is getting worse, or repeatedly returns when activity resumes. A specialist examination can assess foot posture, calf and ankle movement, tendon strength, gait and areas of tenderness. Depending on the findings, imaging such as X-ray, ultrasound or MRI may help identify a stress injury, tendon pathology, soft-tissue lesion or structural issue.
Urgent medical review is appropriate if you have any of the following:
- sudden severe pain with a pop at the back of the ankle or heel
- inability to bear weight after an injury
- redness, heat, marked swelling or fever
- an open wound, unexplained numbness or colour change in the foot
People with diabetes, reduced circulation, neuropathy or immune system conditions should not wait to have a new painful foot problem assessed.
What treatment may involve
Successful treatment starts with matching the plan to the diagnosis, your daily demands and how long symptoms have been present. For plantar fascia and tendon problems, early management may include reducing aggravating activity without stopping all movement, choosing supportive shoes, targeted calf and foot strengthening, and carefully prescribed stretching.
Foot orthoses may be useful where foot mechanics are placing repeated strain on the plantar fascia or tendon. They are not a cure in isolation, and the right design depends on the individual foot and diagnosis. Temporary offloading, heel lifts, strapping or a walking boot may be considered in selected cases, especially where a stress injury or significant tendon irritation is suspected.
Pain relief can assist comfort, but it should not be used to mask pain while returning too quickly to aggravating activity. Anti-inflammatory medicines are not suitable for everyone and should be discussed with a health professional, particularly if you have stomach, kidney, heart or blood-thinning medication considerations.
Some persistent conditions may benefit from shockwave therapy, image-guided procedures or other non-surgical treatments. These options have different evidence, costs, recovery requirements and suitability. A corticosteroid injection, for example, may provide short-term relief for selected plantar fascia presentations, but repeated or poorly targeted injections carry risks including weakening or rupture of the fascia. It is not the right first choice for every painful heel.
When surgery is considered
Most heel conditions improve without surgery, but not all do. Surgery may be discussed when a clearly diagnosed structural problem remains painful and function-limiting after an appropriate period of well-directed conservative care, or where there is a significant tendon tear, bony impingement or other pathology unlikely to settle on its own.
The procedure depends entirely on the cause. It may involve addressing a diseased portion of tendon, removing an impinging bone prominence, releasing a compressed nerve or treating other confirmed pathology. Surgery for plantar heel pain is considered carefully because plantar fascia symptoms can have several contributors, and recovery requires patience and adherence to a structured rehabilitation plan.

Replacing worn-out shoes, avoiding abrupt training changes and maintaining calf strength can reduce the chance of recurrence, but there is no single shoe or exercise that suits every cause.
